Please take a few moments to answer the questions below. We desire that our wonderful puppies goes to wonderful homes!  If you don’t receive a response withing a few days, please email us or call us. Sometimes our responses are lost in Junk Folders.

The information contained in this application is used only for the purpose of placing a puppy/dog and will not be used elsewhere.

Lifestyle:

With these questions, we will be able to match the puppy better with its new home.

Household setting
Do you have children at home?
Does anyone in your household have allergies to animals?
Have you had any dogs before:
Do you have any pets now?
Do you work outside of the home?
Will the puppy live indoors?
If you rent, do you have permission from the landlord to have a dog?
Do you have a fenced yard?
Do you understand the time and energy that it takes to care for a puppy?

For the safety of your puppy, when not in an enclosed area, please keep your puppy on a leash at all times.

Do you have a veterinarian that you plan to use?
Will you be attending obedience classes with your puppy?
What gender are you looking for?
Do you realize that this puppy/dog will be sold as a pet, with Limited AKC Registration, and is not to be used for breeding?
What is the best time to call you?
Upon acceptance of this application, a deposit is required to reserve your puppy. Are you aware that the deposit it non-refundable?
